Patient Portal Access

We invite you to use the IQ Health Patient Portal to access your medical records, review test results, and communicate with your health care Provider.

If you have created a login after 01.01.2024, please click on the first button below to ACCESS your account.

If you have not yet created a login OR created a login prior to 01.01.2024, please click on the second button below to CREATE your new account.
Note that a valid email address is needed to create the account.

If you are having issues with your password or wish to delete your account, please call IQ Health Support at 1-877-621-8014.

Having trouble logging in?

  • If the login page is not accepting your password or you no longer remember it, please select “Forgot Password?” next to the sign-in button and then provide your email or username so the password reset link can be sent.

Accounts created prior to 01.01.2024 are no longer accessible.

  • If you created your account prior to 01.01.2024, PLEASE CLICK ON THE BUTTON BELOW to request an invitation on our new system.
  • Your health information that was stored on our previous electronic records system has been transferred to the IQ Health portal. You can view these transferred records via the Health Record tab under the Visit Summaries category. At the bottom of that page under Other Health Information, your historic health records will be in a file titled “Unknown”. You will be able to download and view this file.
