Check in at the reception desk prior to proceeding to any hospital department or clinic.

If you are coming for your Medicare Annual Wellness Visit, we invite you to print and complete the required forms and bring them with you to your appointment.

Bring your photo ID, insurance card, prescription card, and, if applicable, referral information, medical records, and an updated medication list and bottles.

LATE ARRIVAL & CANCELLATION POLICIES

If you arrive late for your appointment, we may ask you to reschedule to ensure other patient appointments are not delayed.

Please contact our office 24 hours in advance of your scheduled appointment. Failure to do so will require us to mark the visit as a “No Show” appointment.

SURGICAL CHECK IN
PRE-SURGERY

Ask your physician to advise you regarding any medications you take.

If you feel ill or experience a fever, sore throat, cold, intestinal issues, or other symptoms, please contact your surgeon.

Do not take herbal medications for at least fourteen (14) days prior to surgery.

Refrain from smoking a minimum of 12 hours before surgery. Ideally, cut down or stop smoking as soon as possible prior to the day of surgery.

Arrange for someone to drive you to and from surgery.

TWENTY-FOUR HOURS PRIOR TO SURGERY

Bathe or shower the night before surgery but do not shave the surgical area within 24 hours of surgery.

Chewing tobacco, snuff, smoke products, gum, mints, and candy should not be taken or used.

THE DAY OF SURGERY

Leave jewelry and other valuables at home but do bring your insurance card and driver’s license or other form of ID.

Do not wear makeup, artificial nails, or nail polish.

Wear comfortable, loose-fitting clothing and comfortable shoes.

Check in with reception when you arrive at the hospital.

If you normally take blood pressure medication in the morning, take it the morning of surgery with a small sip of water.

If you wear dentures or removable bridgework, remove them before surgery.

Bring all your medications in their original containers.

If you will be staying one night or more...
  • you may bring your own sleepwear, robe, slippers, basic toiletries, and clothes to wear home.
  • and if you use a CPAP/biPAP machine at home, bring it and its accessories with you to use while at the hospital.

FREQUENTLY ASKED QUESTIONS

CAN I REQUEST RECORDS FOR MY CHILD OR SPOUSE WHO IS ILL?

Release authorization is.
  • not required for patients under 18, with the exception of Behavioral Health, STD’s and Pregnancy, which, by law, require permission from a child between the ages of 14-18. In the case of guardians, foster parents, etc., documentation showing proof of guardianship is required.
  • required for patients 18 and over, including spouses, unless there is a POA on file.

CAN I USE A POA TO OBTAIN RECORDS OF A DECEASED RELATIVE?

No. A POA expires upon death. The executor of the estate can request medical records by providing the requred documentation.

CAN I PICK UP COPIES OF MEDICAL RECORDS IN PERSON?

Yes. You may call to arrange a pickup day and then, on that day or after, check in at the SVH reception desk before proceeding to the Medical Records Department. Note that we do not pre-print records; they are prepared upon in-person request once the Authorization of Release form has been completed and submitted.

WHO SHOULD I CONTACT WHEN CALLING FOR MEDICAL RECORDS?

TRANSITIONAL CARE

WHAT IS TRANSITIONAL CARE?

Sierra Vista hospital provides Transitional Care for patients who are no longer acutely ill but are not ready to go home from the hospital where they were treated. Transitional Care is also known as Swing Bed care.
WHAT ARE EXAMPLES OF THE NEED FOR TRANSITIONAL CARE? Common reasons for transitional care include IV antibiotic therapy, complex wound care, rehabilitation after orthopedic surgery, stroke, cardiac procedures, respiratory care, rehabilitation after fractures, amputation, and multiple injuries due to trauma.
HOW DO I QUALIFY FOR TRANSITIONAL CARE?

To qualify for this program, you must need nursing or rehabilitation services that can only be provided in a skilled medical facility.

Examples of skilled care include:
  • Rehabilitation, such as physical therapy, occupational therapy, speech therapy, IV therapy, and respiratory therapy
  • Sterile dressing changes
  • Skin or wound care
HOW DO I GET A REFERRAL?

If your physician determines that skilled care is needed, that physician can refer you to Sierra Vista Hospital. Arrangements will then be made for admission, providing your insurance covers transitional care.

FINANCIAL ASSISTANCE

Sierra Vista Hospital and Clinics serve all patients, regardless of ability to pay. If you cannot afford your co-pay, if you do not have insurance, or are underinsured, we offer access to several financial assistance programs as detailed below.

TO APPLY:

Apply in person with the help of our SVH Financial Counselor or at the Income Support Division at 102 W. Barton Ave. in Truth or Consequences, NM 87901.

Please provide SVH with the Medicaid Approval letter. After you receive your Medicaid card, you must bring it to all appointments.

Please provide SVH with the denial letter and apply for the Indigent Care Program.

INDIGENT CARE

TO APPLY:

Take your completed, signed, and notarized application with supporting documents to the Financial Counselor at Sierra Vista Hospital or deliver them directly to the county office to be given to Indigent Clerk at 1712 N. Date, Truth or Consequences, NM 87901.

Provide SVH with your Indigent Approval letter. NOTE: Indigent status is good for one year.

IF YOU ARE DENIED

Please provide SVH with the denial letter and apply for the Indigent program.

Charity Care

TO APPLY:

You must first be denied Medicaid, or your outstanding balances must be outside of the Medicaid approval dates. You must also be denied or be unqualified for Indigent Care to apply.

Complete the Charity Care application and provide all requested documentation. The program is based on Federal Register for Poverty Table guidelines. A determination will be made, and you will be notified of approval or denial. Your Charity discount may range from 25% to 100%.

PAYMENT PLAN

TO APPLY:

Make an appointment with the SVH Financial Counselor to set up payment arrangements. Please call 575.743.1215.

PAYMENT PLAN TERMS

Balances must be paid within 12 months. If payments are not received as stipulated and no other arrangements have been made, payment plan shall be in breach and the entire balance will be due and payable on demand. If full payment is not received within 30 days of the breach, the account will be turned over to a collection agency and the responsible party must then deal directly with the collection agency.

SLIDING FEE DISCOUNT

OVERVIEW

The Sliding Fee Discount Program provides under-insured and un-insured clinic patients with healthcare at a lower cost. Services outside of the clinics, such as labs and radiology, are not included in this program.

Eligibility is based on your monthly income and the number of persons you claim on your annual income tax report. If you are approved, discounts are valid for one year. Applicants are required to re-apply when their financial situation changes. The Sliding Fee Scale is reviewed in accordance with the fiscal accounting year.

TO APPLY:

Medical Hardship is weighed on a case-by-case basis and/or when all Medical Assistance programs have been denied. To qualify for Medical Hardship, you must complete the application and present it to us together with ALL requested documentation. A checklist is included with the application to help you gather the needed documents.
